Provisional Ballot Cure Affidavit

If the Osceola County Supervisor of Elections Office contacted you because the Provisional Ballot you voted was either:

missing the signature
signature provided did not match the signature on the voter registration file

You will be required to complete a Provisional Ballot Cure Affidavit and provide a copy of one of the approved identifications listed on the affidavit.

The completed Provisional Ballot Cure Affidavit and a copy of the identification (refer to list on affidavit) must be returned to the Osceola County Supervisor of Elections office no later than 5 p.m. on the second day after the election. You may submit the affidavit and a copy of the identification to the Supervisor of Elections office:
